Service Academy Nomination Application

Applicants are responsible for registering for the SAT or ACT and requesting the scores be sent to your nominating representatives and perspective academies. My office's SAT code is #6173 / ACT code is #7312.

Select your preference of academies — with "First" being your highest preference. Please rank only the Academies to which you intend to apply.

In the case that your school will not release any materials needed to complete your application please have the school's guidance counselor email the material to himesserviceacademy@mail.house.gov.

Please upload your transcript with an official seal or stamp, in a PDF format. Your total combined file size cannot exceed 1MB. Documents must be the following file types: .pdf

Please write about your desire to attend a United States service academy. Perhaps you may wish to describe the event(s) that made you interested in attending a service academy. Furthermore, you may want to write about a family member or friend who was influential in your decision-making. Please be clear in describing why attending a U.S. service academy is right for you.
